us for medical examinations should liaise with the secretary regarding
appointment times. Please see our map for how to find us

Please bring any paperwork regarding the accident and also
the form we sent you for details of yourself , the accident and how it affected
you. You may complete the form on line, bearing in mind that it is sent by non
encrypted email, or if you wish complete the form and print it out and post it or
bring it with you.
