How do I request a sicknote?
For an initial sick certificate it is important that you have
been seen by a doctor or triage nurse.
The first 7 days of an illness are covered by a self certificate
form available from your employer, and you do not need to see a doctor in this
period for a sick certificate.
Certain employees insist on a certificate in this initial 7 day
period. By law they should accept a self certificate but if they insist we are
only able to issue a private certificate in these first 7 days of illness.
Requests for repeat sick certificates should be made by phoning 01733
366688 ( our prescription request line), giving 48 hours notice for
collection.
